We are actively seeking a Director of Chronic Care Management and looking for an RN who is ready to roll up their sleeves and guide our Chronic Care Team to success. This role is responsible for designing, implementing, optimizing, and scaling comprehensive care management initiatives that improve outcomes for patients with complex and chronic conditions.

The Director will lead interdisciplinary teams, ensure regulatory compliance (including CMS CCM requirements), drive quality and performance improvement, and collaborate cross-functionally to enhance patient engagement, care coordination, and value-based care performance. What You’ll Bring:

  • Minimum of five (5) years progressively responsible management and staff development experience in senior living
  • Current and Valid Registered Nurse (RN) license for the state of Illinois
  • Strong leadership and the ability to hold those under your direction accountable for results
  • Strong knowledge of CMS Chronic Care Management requirements and value-based care models.
  • Knowledge of MDS and quality improvement
  • A valid driver’s license as travel is required and may be 60 miles or greater
