Chestnut Family Health Center is committed to providing high-quality health services to our community. We are seeking a dedicated and compassionate Driver to join our team and help us transport patients safely and efficiently. Chestnut Family Health Center is a part of Chestnut Health Systems.

Responsibilities

  • Safely transport patients to and from the Chestnut Health Systems for treatment and medical appointments.
  • Coordinate transportation and communicate any patient/client needs with staff.
  • Monitor and report any vehicle maintenance issues to the maintenance department.
  • Maintain accurate documentation of all services provided within required time frames.
  • Function as a team member, attend meetings and patient staffing as needed, and participate in program development activities.
  • Meet or exceed productivity expectations as determined by your supervisor.
  • Coordinate services with the treatment team and other Chestnut programs to ensure continuity of care.
  • Demonstrate proficient computer skills to perform job duties.
  • Uphold professionalism and appropriate boundaries with patients/clients.
  • Support and model recovery values.
  • Complete and maintain CPR, Crisis Prevention and Intervention (CPI), and vehicle safety certification.
  • Show knowledge of and sensitivity to individuals' cultural beliefs and values.
  • Promote Chestnut's culture of customer service excellence by adhering to our behavioral standards.
  • Maintain and protect patient confidentiality and the security of all organizational information.

Qualifications

  • High school diploma or equivalent.
  • Valid driver's license with a good driving record and qualifications for agency auto insurance coverage.
  • Ability to maintain professional boundaries and interactions with patients.
  • Basic computer skills, including MS Word, MS Excel, Outlook, Adobe Acrobat, and electronic medical records (EMR).
